In a bid to bring about a radical change in the spheres of education and health, NITI Ayog introduced SATH – Sustainable Action for Transforming Human Capital, a programme crafted in collaboration with the state governments. This initiative was born out of the numerous requests from various states for technical assistance from NITI Ayog. SATH stands as a testament to the agenda of cooperative federalism. The responsibility for the execution of the programme lies with NITI Ayog, McKinsey & Company, and IPE Global consortium, which were selected through a rigorous competitive bidding process. The programme is established on a tripartite partnership between the state governments, NITI Ayog, and a knowledge partner unique to each sector.

Through SATH, NITI Ayog aspires to create three exemplary states in terms of health systems. These selected states will receive assistance in drafting a comprehensive blueprint, devising a structure for program governance, developing a monitoring & tracking mechanism, and providing support to the state institutions to realize the ultimate objectives. NITI Ayog has articulated a three-stage process for the selection of states: expression of interest, state presentations, and evaluation based on commitment to health sector reforms. After an intense competitive process based on impact potential and probability of success, the states of Karnataka, Uttar Pradesh, and Assam were chosen. Criteria such as malaria incidence, IMR, MMR, and others were taken into account to assess potential impact, while factors like compliance with Indian Public Health System norms, and the density of doctors and nurses were used to gauge the likelihood of success.

The chosen states are expected to pledge to governance reforms in both the health and education sectors within a specific timeframe. A programme management unit will be established in these three states for a period of thirty months to ensure the effectiveness and efficiency of the governance structures. NITI Ayog envisions to build a model that can be seamlessly replicated and adopted by other states, thereby bringing about a significant transformation in these sectors.
